This programme is unable to support projects or activities that are taking place in Derby City.

In addition to providing details about your group and the project/activity you are seeking funding for; you will also need to supply the following financial information:

• Organisation contact details
• A brief description of your organisation
• Information about your smokefree plans
• Confirmation that your organisation can meet the grant requirements
• Bank account details
• A simple funding breakdown

Please ensure all questions are fully answered, as incomplete applications may result in delays.
